Required Preschool Forms
All forms must be completed and in your child's file BEFORE attending school. Documents may be mailed, emailed, or brought to the school during regular business hours.
2026-2027 Forms
Child Admission Form - Complete in family profile after registering.
Health Statement - Print and have your child's physician sign.
Immunization Records - Please submit an updated copy from your childs physician.
Allergy Emergency Plan - If your child has a diagnosed food allergy, please provide a written Allergy Emergency Plan from your child's physician.

All paperwork must be on file before your child attends camp; documents may be mailed, emailed, or delivered to the school during regular hours.
Child Admission Form – Complete online in your family profile after registering.
Immunization Records/Health Statement – Submit a copy of your child’s immunization records and a signed Health Statement, or indicate on the admission form if they are on file with another school.
